It&#8217;s<em><strong>\u00a0<\/strong><\/em>a pathological condition where the testicles\u00a0start off healthy, but gradually reduce in size. This condition could be caused by age, injury, inguinal hernia\u00a0or medical condition such as autoimmune illness.\u00a0<strong>It is usually accompanied by a loss of function.\u00a0<\/strong><\/p>\n<p>It\u00a0is a common side effect reported in the research, with many bodybuilders having smaller testes comparable to\u00a0&#8216;pre-pubertal boys\u2019 [1].<\/p>\n<p>The testicles\u00a0are made up of mostly two cell types &#8211; <em><strong>Leydig cells and germ cells<\/strong><\/em>. The function of the Leydig cell is to <em><strong>produce testosterone<\/strong><\/em>, and the germ cell produces sperm. If cell production from these sites is even, then the testes remain full, firm and round. When cell production decreases &#8211; in either cell type &#8211; testicular atrophy can occur.<\/p>\n<p><img decoding=\"async\" class=\"aligncenter wp-image-1190 size-full\" src=\"https:\/\/www.testofuel.com\/tf\/wp-content\/uploads\/2016\/07\/Ball-Shrinkage-and-Steroids.jpg\" alt=\"shrunken balls on steroids\" width=\"800\" height=\"451\" srcset=\"https:\/\/www.testofuel.com\/tf\/wp-content\/uploads\/2016\/07\/Ball-Shrinkage-and-Steroids.jpg 800w, https:\/\/www.testofuel.com\/tf\/wp-content\/uploads\/2016\/07\/Ball-Shrinkage-and-Steroids-150x85.jpg 150w, https:\/\/www.testofuel.com\/tf\/wp-content\/uploads\/2016\/07\/Ball-Shrinkage-and-Steroids-300x169.jpg 300w, https:\/\/www.testofuel.com\/tf\/wp-content\/uploads\/2016\/07\/Ball-Shrinkage-and-Steroids-768x433.jpg 768w\" sizes=\"(max-width: 800px) 100vw, 800px\" \/><\/p>\n<hr \/>\n<div class=\"group infobox infobox-blue mtop20 mbottom20\">\n<p><span style=\"text-decoration: underline;\"><em><strong>Key Point:<\/strong><\/em><\/span>\u00a0Testicular atrophy is when your\u00a0testicles\u00a0shrink in size and lose firmness and overall shape.<\/p>\n<\/div>\n<hr \/>\n<h2>Anabolic steroid use and testosterone<\/h2>\n<p>In order to get the most from this section\u00a0you&#8217;ll need to know a few bits of terminology &#8211;\u00a0we presume that you&#8217;re familiar with the actions and mechanisms of steroids<i>, <\/i>but if you&#8217;re not then<i>\u00a0we\u00a0<\/i>suggest you read about them <strong><a href=\"https:\/\/www.testofuel.com\/tf\/how-do-anabolic-steroids-work\/\">here<\/a><\/strong>.<\/p>\n<p>The use of steroids, or<em><strong> A<\/strong><\/em><em><strong>ndrogenic-anabolic Steroids (AAS)<\/strong><\/em>, is commonplace and\u00a0widespread in both amateur and professional athletes [2].\u00a0It is not without side effects.<\/p>\n<p>One of the\u00a0side effects which is often reported anecdotally and in the research is that of <strong>smaller testes through prolonged steroid use.<\/strong><\/p>\n<p>The way that testosterone production is regulated in the body is via the <strong>hypothalamic-pituitary-gonodal axis (HPG)<\/strong>.\u00a0This aspect of the\u00a0reproductive system is made up of the hypothalamus and the pituitary glands &#8211; located in the brain, and the testes.<\/p>\n<p>The HPG system works on a \u2018feedback loop\u2019 \u2013 what that means is that if your body isn\u2019t producing enough T then the hypothalamus stimulates you to produce more, and likewise if you produce too much it suppresses production.\u00a0When you take steroids\u00a0you\u2019re levels of T are so high that the <strong>hypothalamus sends signals to the testicles\u00a0telling them you need to produce your own anymore.\u00a0<\/strong><\/p>\n<p>By dumping so much exogenous T into your body &#8211; as occurs with AAS use, your testicles\u00a0respond by essentially becoming dormant &#8211; and this can result in a number of side effects including loss of libido, loss of erection and of course &#8211; smaller testes.<\/p>\n<hr \/>\n<h2>Studies and case reports &#8211;\u00a0will steroids shrink your balls?<\/h2>\n<p>Most of the data on bodybuilders, steroids and testicular atrophy\u00a0comes from case reports rather than longitudinal or cohort studies.<\/p>\n<p>For example, a case report of a 30 year-old competitive bodybuilder [3]\u00a0who died after collapsing at home hours after a weight training session, found a number of side effects of prolonged steroid use &#8211; abnormal muscle development, liver issues, and\u00a0<em><strong>testicular\u00a0atrophy.\u00a0<\/strong><\/em>He had been using AAS for 6 months.<\/p>\n<p>&#8230;and it&#8217;s not just individual case reports either &#8211; there are a number of larger reports that show adverse effects:<\/p>\n<p>One study [4]\u00a0recruited\u00a0100 steroid using athletes in order to identify via questionnaire, their regimens and side effects. Out of all volunteers, 88% reported experiencing steroid related side effects &#8211; with\u00a0<strong>40% reporting testicular\u00a0atrophy as a subjective side effect. <\/strong>This was one of the most reported side effects, second only to acne.<\/p>\n<p>Similarly, in a large study of 207 bodybuilders with a history of steroid use [5], numerous adverse side effects were reported &#8211; including acne, gynecomastia, abnormal hair growth and increased blood pressure. <strong>8% of those questioned reported testicular atrophy &#8211; that&#8217;s 17\u00a0people\u00a0out of the 207.<\/strong><\/p>\n<hr \/>\n<p><img decoding=\"async\" class=\"aligncenter wp-image-1189 size-full\" src=\"https:\/\/www.testofuel.com\/tf\/wp-content\/uploads\/2016\/07\/Steroids-and-Testicular-Atrophy.jpg\" alt=\"Steroids-and-Testicular-Atrophy\" width=\"800\" height=\"450\" srcset=\"https:\/\/www.testofuel.com\/tf\/wp-content\/uploads\/2016\/07\/Steroids-and-Testicular-Atrophy.jpg 800w, https:\/\/www.testofuel.com\/tf\/wp-content\/uploads\/2016\/07\/Steroids-and-Testicular-Atrophy-150x84.jpg 150w, https:\/\/www.testofuel.com\/tf\/wp-content\/uploads\/2016\/07\/Steroids-and-Testicular-Atrophy-300x169.jpg 300w, https:\/\/www.testofuel.com\/tf\/wp-content\/uploads\/2016\/07\/Steroids-and-Testicular-Atrophy-768x432.jpg 768w\" sizes=\"(max-width: 800px) 100vw, 800px\" \/><\/p>\n<hr \/>\n<div class=\"group infobox infobox-blue mtop20 mbottom20\">\n<p><span style=\"text-decoration: underline;\"><em><strong>Key Point:<\/strong><\/em><\/span> Research shows that anabolic steroids shrink the testes due to decreased T\u00a0production by the Leydig cells<\/p>\n<\/div>\n<hr \/>\n<h2>Other side effects<\/h2>\n<p>According to the journal <em>Urology<\/em> [1],<span style=\"color: #ff00ff;\">\u00a0<\/span><strong>Exogenous testosterone typically results in atrophy of the germinal cells in normal men with varying degrees of suppression of spermatogenesis.\u00a0<\/strong><\/p>\n<p>When higher doses of AAS are taken you can expect your semen levels (quality and quantity) to take a nose dive, and this may lead to infertility in later life [6].<\/p>\n<p><em><strong>As T levels plummet and testes shrink, you can also expect that your ability to get an erection and your overall libido can also fall.\u00a0<\/strong><\/em><\/p>\n<p>For example, a report on high-dose steroid use in 30 bodybuilders\u00a0in the journal\u00a0<em>Life Sciences <\/em>[6], found that only <strong>17.7% of the volunteers had normal sperm levels<\/strong>.<\/p>\n<hr \/>\n<h2>Will your testes grow back in size?<\/h2>\n<p>It is worth noting that in <strong>most cases<\/strong>, shrunken balls are\u00a0<strong>temporary<\/strong> &#8211; when you&#8217;re HPG feedback system realizes that you&#8217;re not flooding your body with AAS anymore it will start to produce its own again, allowing the testes to become firmer and larger once again.<\/p>\n<p>It is worth noting however that the time taken for full recover could be as long as a few months and whilst the changes in these hormones are reversible after discontinued use,\u00a0<strong>the long-term effects on the hypothalamus-pituitary-testicular axis are still unknown <\/strong>[6].<\/p>\n<p>It is very much a process of &#8216;the bigger the dose, the longer the recovery&#8217;.
