Otherwise known as\u00a0<strong>HMG-CoA reductase inhibitors,\u00a0<\/strong>they are a class of medication that lower lipid levels in the blood, and as such,\u00a0<strong>greatly reduce cardiovascular-related morbidity and mortality in patients with and without coronary artery disease\u00a0<\/strong>[1].<\/p>\n<p>According to the <em>US\u00a0Department\u00a0of Health and Human Services\u00a0<\/em>[2], <strong>use of this drug\u00a0rose from 18% to 26% between 2003 and 2012<\/strong>. In 2013, cholesterol treatment guidelines were changed in order to recommend HMG-CoA reductase inhibitors, in addition to general lifestyle changes, to treat high-cholesterol cardiovascular disease.<\/p>\n<p>There are a number of drugs\u00a0available on the market. These include <em><strong>simvastatin, pravastatin<\/strong> <\/em>and <em><strong>atorvastatin<\/strong> <\/em>amongst many others.<\/p>\n<p>Whilst this medication is widely used throughout the US and worldwide, there has been controversy relating to\u00a0potential side effects. <strong>Common side effects include headache, drowsiness, nausea, abdominal bloating and diarrhea.\u00a0<\/strong><\/p>\n<p>In some\u00a0cases, this medication can cause\u00a0<em><strong>myopathy\u00a0<\/strong>&#8211; <\/em>muscle pain [3] &#8211; or in rare cases, extreme muscle inflammation and damage &#8211; referred to as <em><strong>r<\/strong><\/em><b><em>habdomyolysis<\/em>. <\/b>This condition can make your\u00a0muscles become painful and weak &#8211; a little bit like the delayed soreness you get from a big weights session, but more severe. However, this action\u00a0<strong>could potentially decrease testosterone levels by reducing the availability of cholesterol for androgen synthesis<\/strong>\u00a0[4].<strong>\u00a0<\/strong><\/p>\n<p>Testosterone and cholesterol are both classed as steroid hormones due to their structure. There aren&#8217;t as many that analyze its effects on T levels, but here is a breakdown of the most important ones:<\/p>\n<p>In a\u00a0large study of 3,484 men suffering from erectile dysfunction [6], <strong>both total and free<\/strong>\u00a0<strong>T levels were found to be significantly lower in those taking statins<\/strong>. On top of that, the use of cholesterol-lowering medications was associated with smaller testes and decreased\u00a0secondary sex hormones.<\/p>\n<p>The authors of the study suggested that this drug may be a cause of\u00a0<em><strong>hypogonadism<\/strong> <\/em>&#8211; low testosterone levels characterized by an absence or reduction of hormones in the gonads.<\/p>\n<p>In a\u00a0paper from the <em>University of California\u00a0<\/em>[7], over 1000 men and women volunteers were prescribed\u00a0either cholesterol-lowering drugs\u00a0or placebo over a 6-month period. The researchers primarily waned to assess changes to aggression, but also measured\u00a0T levels as well.<\/p>\n<p><em><strong>The study found that whilst women volunteers saw an increase in aggression, males saw significant reductions.\u00a0<\/strong><\/em><\/p>\n<p>Additionally (and possibly even the cause of reduced aggression), <strong>total T levels were lower in those that were prescribed pravastatin and even lower in those who were asked to use simvastatin.\u00a0<\/strong><\/p>\n<p>Two years previous to this study, the same author published a similar study, this time in\u00a0<em>Circulation<\/em>\u00a0[8],<em>\u00a0<\/em>detailing the effects of testosterone relative to lipid changes whilst taking HMG-CoA reductase inhibitors.<\/p>\n<p>In this study, 694 men were asked to take one of two types of cholesterol-lowering drug\u00a0or a placebo and observed over a 6-month period. The results found that <strong>simvastatin\u00a0had a significant lowering effect on testosterone compared to placebo<\/strong>.<\/p>\n<p>Lastly, cholesterol-lowering drugs have also been found to reduce male androgen levels in type 2 diabetics [4].
